The Salvation Army is seeking a Transition Coach to support young people leaving care in Western Australia from Morley, WA. The role is full-time and based in the community, offering practical coaching, mentoring and coordinated support to help youths transition to independence.

You will work with Child Protection staff and carers, delivering trauma-informed, culturally protected care and helping clients access housing, education, training and employment opportunities.
