As aGrade 2 Social Worker, you'll work with adults experiencing complex health and social needs, delivering high-quality psychosocial assessment, counselling, advocacy and discharge planning. Working alongside a collaborative multidisciplinary team, you'll help achieve safe, person-centred outcomes for patients and their families.

You'll thrive in this role if you:
