About the Role
Chosen Family isseekingexperienced and compassionate Registered Nurses (RNs) to provide clinical nursingsupportsand disability services to participants across home, community, respite, and SIL settings.
The RN will play a key role in delivering and overseeing complexsupportswhile ensuring participants receive safe, high-quality, and person-centredcare tailored to their individual goals and needs.
Key Responsibilities
Clinical Nursing Supports
- Conduct comprehensive nursing assessments and participant reviews
- Deliver complex clinical supports including:
- PEG feeding
- Tracheostomy care
- Diabetes management
- Enteral feeding
- Medication administration and management
- Continence assessments and care
- Monitor participant health status and clinical outcomes
- Identifychanges in participant condition and escalat(e?) appropriately
- Respond to medical incidents and emergency situations asrequired
- Ensure infection prevention and control standards are maintained
Care Planning & Clinical Oversight
- Develop, implement, and review participant care plans
- Conduct clinical risk assessments and health reviews
- Provide clinical oversight to support workers and Enrolled Nurses
- Ensure secure delivery ofhigh-intensitysupports
- Participate in incident management and safeguarding processes
- Support staff training and competency assessments whererequired
- Promote evidence-based and best-practice nursing care
- Support positivebehavioursupport implementation whererequired
- Promote participant dignity, independence, and choice
- Support participants to achieve individual goals and improve quality of life
- Encourage community participation and social inclusion
- Deliver trauma-informed and culturally responsive care
- Build therapeutic and professional participant relationships
- Support participant emotional wellbeing and health literacy
Stakeholder Engagement
- Liaise with families, guardians, Support Coordinators, and allied health professionals
- Collaborate with multidisciplinary teams to ensure continuity of care
- Maintain professional and compassionate communication
- Participate in case conferences and clinical reviews
- Support positive relationships with participant support networks
Compliance & Documentation
- Maintainaccuratenursing and clinical documentation
- Complete medication charts, incident reports, and progress notes
- Ensure compliance with NDIS Practice Standards and clinical governance frameworks
- Participate in audits, reviews, and quality improvement initiatives
- Maintain participant privacy and confidentiality
- Ensure compliance withorganisationalpolicies and legislative requirements
- Bachelor of Nursing
- Current AHPRA Registration as a Registered Nurse
- Current NDIS Worker Screening Check
- Current First Aid & CPR
- Strong clinical assessment and documentation skills
- Valid Driver’sLicence
- Disability nursing experience
- Experience within SIL or complex care environments
- High-intensity support competency
- Experience within community or home care settings
