Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting. We also support people, including older people and people with disability, to stay living at home longer and to live well in their community.
Our broad range of primary health services, care management and in home and community support services can be tailored to an individual’s needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

Job Description
The opportunity: We are looking for a Dietitian with at least 3 years of experience to join our supportive and professional multidisciplinary team in a flexible part time capacity.
Responsibilities
- Provision of high-quality Dietetic services to all healthAbility clients from various funding sources.
- Active participation in regular interdisciplinary team meetings, service planning and reviews.
- Contribute to the development and innovation of integrated multidisciplinary service delivery models.
- Ensure that care services are accessible, culturally relevant and inclusive.

Desired Skills and Experience
- 3 years or more of clinical experience in relevant areas preferably in a community health setting.
- Experience in developing, implementing and evaluating group and individual programs.
- An understanding of chronic disease management and models which promote self-management.
- Ability to work with a high level of independence and autonomy and accountability.
