Social Worker Level 3, Acute Services Team, Prince of Wales Hospital
Position Summary:
We are inviting applications for a Social Worker Level 3 in the Acute Services Social Work Team. The successful applicant will provide social work services to patients and their carers across acute inpatient areas. The role involves counselling, crisis intervention, bereavement support, referral facilitation, discharge planning, and advocacy within a multidisciplinary team.
Responsibilities
- Providing counselling and support to patients and families adjusting to illness and hospitalisation.
- Delivering crisis intervention and bereavement support.
- Facilitating referrals to community agencies and support services.
- Undertaking discharge planning in collaboration with patients, families, and the multidisciplinary team.
- Advocating for patients and supporting complex care coordination.
Qualifications
- Registered social worker with eligibility for membership of the AASW.
- Schedule C of the NSW Health Service Health Professionals (State) Award compliance.
- Extensive post‑graduate clinical experience in a relevant specialty area.
- Excellent negotiation, decision‑making, and advocacy skills with the ability to interact constructively with a diverse range of stakeholders.
- Capacity to provide in‑depth advice and educational support to enhance service delivery and to guide and support others.
- Teamworking skills and advanced reasoning abilities for novel, complex, or critical situations.
- Experience in initiating, leading, completing, and sharing quality improvement initiatives and service evaluation processes.
- Planning and prioritising own and team work effectively within agreed time frames.
- Experience in crisis interventions and participation in after‑hours crisis on‑call services.
