Arche Health is a not-for-profit community-based organisation that delivers high quality support services to the local community. We deliver the Integrated Team Care (ITC) program which is designed to improve health outcomes for Aboriginal and Torres Strait Islander people living with chronic conditions such as diabetes, cardiovascular disease, chronic respiratory disease, renal disease, cancer, and mental health conditions.

The program provides care coordination, access to multidisciplinary health services, and culturally safe support to help clients manage their conditions effectively.

We require a Care Coordinator (Full Time) who has excellent communication, community engagement and administration skills to provide holistic care coordination for clients in our busy Aboriginal Health Team as part of the Integrated Team Care (ITC) Program.

This position is based in Bentley and works closely with GPs, health practitioners, clients and other key stakeholders in the Perth Metro – North West and South East regions, to achieve better health outcomes for our Aboriginal and Torres Strait Islander clients through better access to culturally appropriate health services.

ROLE

The Care Coordinator will assist and support the Arche Health Aboriginal Health Manager to improve health outcomes for Aboriginal and Torres Strait Islander people living with chronic health conditions through better access to high quality, culturally appropriate, coordinated care through the Integrated Team Care (ITC) Program.





The position will work collaboratively with members of a multidisciplinary team, General Practitioners and internal and external stakeholders to ensure the ITC Program is delivered in an productive and professional manner, in line with the contractual requirements.

DUTIES & RESPONSIBILITIES

- Support clients to access the services and supports outlined in their GP Chronic Condition Management Plan (GPCCMP).
- Improve clients access to culturally appropriate mainstream and community-controlled health services.
- Coordinate clients' medical and allied health appointments and arrange transport.
- Liaise with clients' family and/or their carers, as appropriate.
- Encourage clients to attend regular health care appointments.
- Improve clients' chronic disease self-management skills, through:
- Adhering to treatment regimens – e.g., taking medication regularly.
- Monitoring of chronic conditions – e.g., regular blood pressure monitoring.
- Connecting with appropriate community-based services – e.g., My Aged Care providers.
- Promote the ITC Program to Aboriginal and Torres Strait Islander clients residing in the Arche Health catchment regions.




- Assist the ITC Program team to identify barriers that impact on Aboriginal and Torres Strait Islander people's access to health services and work to implement meaningful solutions.
- Contribute to developing culturally appropriate health promotion material.
- Liaise and collaborate with hospital and health care staff, GPs, Arche Health staff and other health providers to improve the delivery of culturally appropriate care for clients.
- Contribute as an effective team member at Arche Health team meetings and represent Arche Health on appropriate committees as required.

SELECTION CRITERIA – QUALIFICATIONS, SKILLS, KNOWLEDGE, EXPERIENCE

- Eligible for registration as an Enrolled Nurse by the Nursing and Midwifery Board of Australia; and/or Certificate IV Aboriginal Health Care, as well as registration with AHPRA as an Aboriginal Health Practitioner – essential.
- Previous experience in care coordination, highly desirable.
- Previous experience working with Aboriginal and/or Torres Strait Islander people, highly desirable.
- Knowledge of the social determinants of health and issues affecting Aboriginal and/or Torres Strait Islander people, essential.
- Sound understanding and knowledge of general practice, primary health care and Aboriginal and Torres Strait Islander health issues.
- Highly developed interpersonal skills, including demonstrated liaison and negotiating skills.
- Ability to set priorities, plan and act to achieve desired outcomes within specified timeframes.
- Current Driver's Licence.
- Working with Children's Check.
