Job Opportunity

Part‑time or Full‑time positions at St Vincent’s Care at Home (SVCAH), located in Sydney Private Hospital, Darlinghurst.

Responsibilities

- Work collaboratively with a close‑knit multidisciplinary team, including MCAH Leads, Visiting Medical Officers, St Vincent’s Care Facilities and other allied health staff.
- Assess patient needs, develop tailored care plans, and coordinate safe discharge.
- Deliver advanced nursing interventions, including IV therapies, complex wound care and monitoring of complex conditions.
- Manage and coordinate new referrals, escalating care when required.
- Promote patient independence and enhance quality of life, strengthening the bridge between hospital and home care.
- Provide guidance and mentorship to new staff members within the growing team.

Qualifications

- Registered Nurse with AHPRA.
- At least 3 years post‑graduate nursing experience in community, medical‑surgical or critical care settings.
- Solid interpersonal and communication skills to build rapport quickly.
- Availability to work a variety of shifts, including weekends and overnight on‑call (telephony only).
- NSW driver’s licence with no restrictions.
