About the Role

The Palliative Care Social Worker will provide support for people with a life-limiting condition and their families within Murrumbidgee Local Health District. The Social Worker is an integral part of the MLHD Specialist Palliative Care Team, working alongside patient liaison nurses to provide multidisciplinary social work input, enhance early palliative care services, and ease transition of care.

Responsibilities

- Providing psychosocial assessment, counselling, information and advocacy to clients, patients and their families/carers within a client-centred framework.
- Ensuring client care is delivered within professional, organisational, legal and ethical boundaries and reflects evidence-based best practice.
- Liaising with care and service providers to ensure a seamless continuum of care that meets the needs of clients, patients and their families/carers.
- Providing appropriate social work interventions and support for consumers of the Palliative Care service during periods of high family stress, including grief and loss counselling.
- Maintaining accurate client records, databases and statistics,



completing work and reports in a timely manner and meeting all prescribed Key Performance Indicators.
- Supporting the training of Social Work students on clinical placement and providing supervision to AHA and Level 1 staff as required.

Our ideal candidate will demonstrate

- A Social Work degree providing eligibility for membership of the Australian Association of Social Workers (AASW) and a minimum of three years' clinical experience in a health-related field.
- Experience providing a range of social work interventions,



with a sound understanding of the challenges associated with life-limiting illness, end of life care, and grief and loss.
- High level written and verbal communication, negotiation and interpersonal skills, with confidence conducting consultations via virtual and telehealth platforms.
- A commitment to clinical supervision, ongoing professional learning and evidence-based practice within social work.
