About us:

The Cairns Youth Step up Step Down service operates as an integrated model, where the Cairns and Hinterland Hospital and Health Service provides clinical services alongside provision of non-clinical support services by Stride, Non-Government Organisation. The model provides a step up from community to prevent further deterioration of a young person's mental state and reduce the likelihood of an acute mental health inpatient admission. Conversely, the step down enables shorter admissions at acute mental health inpatient units.

About your role:

In this position you will…..

- Work collaboratively with consumers and multidisciplinary professionals to provide clinical care and a therapeutic environment conducive to the dignity and well-being of consumers and carers within the mental health service.
- Participate as a member of the Youth Step up Step Down team to undertake primary responsibility for the delivery of clinical services to the consumer, including intake, ongoing assessment and treatment, admission and discharge, medication management, liaison with referring agents,



discharge planning and evaluation of care using appropriate outcome and data measurement systems and recovery focused interventions.
- Work with families and client's communities to promote recovery and community inclusion, and to access generic supports, opportunities and resources.
- Engage in incident reporting/investigation where required and conduct quality improvements audits and support the development of risk minimisation strategies/activities to promote safe and /or rectify unsafe practice.
