Remuneration:
$79,368.82 – $111,427.34 per annum, pro rata (+ super + 17.5% leave loading where applicable)
About the Opportunity
Step into a rewarding role as an Integrated Care Coordinator and help improve outcomes for people living with chronic and complex health conditions across the Corowa region. As a Registered Nurse, you'll coordinate person-centred care across hospitals, general practice, community health and social support services, working closely with patients, families and multidisciplinary teams to deliver seamless, integrated care.
About You
- Current registration as a Registered Nurse with AHPRA and completion of a Graduate Nurse Program or equivalent.
- Experience in comprehensive assessment, care planning and coordinating care for people with chronic or complex health needs.
- The ability to work independently while collaborating effectively within multidisciplinary teams and across healthcare services.
- Excellent communication, organisational and relationship-building skills, with a commitment to patient-centred care.
- A sound understanding of quality, safety and continuous improvement, with a reflective approach to skilled practice.
- A current NSW Driver Licence and willingness to travel across the local service area as required.
