About the Role
As the Community Connector Team Leader, you will lead a dedicated team delivering culturally safe navigation, advocacy, and coordination services across aged care, NDIS, health, and community support systems.
You will help ensure community members can access the services they need while maintaining choice, independence, and self-determination. You'll also build strong partnerships with service providers and advocate for improved outcomes for the community.
Key Responsibilities
- Lead, mentor, and support the Community Connector Team.
- Oversee delivery of the NACCHO Elder Care Support Program and NDIS Remote Community Connector Program.
- Support Elders and people with disability to navigate complex service systems.
- Advocate for consumers and families to access appropriate supports.
- Build strong relationships with community members, service providers, and government agencies.
- Monitor program performance, quality improvement, and compliance requirements.
- Promote culturally safe, person-centred service delivery.
About You
You are an experienced people leader who is passionate about community empowerment and improving access to services for Aboriginal people.
Essential Requirements
- Qualification in Community Services, Health, Human Services, or a related field.
- Experience working with Aboriginal and Torres Strait Islander communities.
- Previous leadership or staff supervision experience.
- Experience in aged care, disability, health, or community services.
- Strong communication, advocacy, relationship-building, and organisational skills.
- Current Driver's Licence.
- Ability to obtain and maintain required clearances.
Highly Regarded
- Experience working in remote Aboriginal communities.
- Knowledge of My Aged Care, NDIS, and Commonwealth aged care programs.
- Understanding of Pitjantjatjara language and culture.
- Experience in quality, accreditation, and program reporting.
