As a Senior Support Coordinator, you will manage a diverse caseload

while working alongside an experienced leadership team committed

to delivering exceptional participant outcomes.

Your responsibilities will include:

- Coordinating and implementing participant NDIS plans
- Supporting participants with complex support needs and life

transitions

- Developing solid working relationships with participants, families,

guardians and carers

- Collaborating with the NDIA, hospitals, allied health professionals,

plan managers, community organisations and other service providers

- Preparing participants for NDIS plan reviews and advocating for

appropriate supports

- Identifying and responding to risks while promoting participant

choice, control and independence

- Maintaining accurate, timely and professional documentation, case

notes and reports

- Building sustainable support networks that achieve long-term

participant outcomes

Pay: $100,000.00 – $115,000.00 per year
