Our client Prestige Disability Services is seeking a dedicated and compassionate Community Worker to join our growing team in Australia. This is a full-time position supporting individuals, families and communities by delivering person-centred services that promote independence, wellbeing and community participation.

Key Responsibilities

As a Community Worker, you will be responsible for:

· Assessing the needs of individuals, families and communities to identify appropriate support requirements.

· Planning, coordinating and implementing individual support plans and community-based programs.

· Conducting client assessments to determine the nature and extent of social, emotional and practical support needs.

· Providing information, guidance, advocacy and ongoing support to clients and their families.

· Monitoring client progress, reviewing support plans and evaluating service outcomes.

· Referring clients to specialist services, healthcare providers and government agencies where appropriate.

· Liaising with community organisations, local service providers and government agencies to improve client outcomes.

· Promoting community resources, support services and social inclusion initiatives.

· Supporting people with disabilities, older persons, families, children, young people and other vulnerable members of the community.

· Assisting clients to overcome social, emotional, financial and practical challenges while encouraging independence and community participation.

· Identifying community needs and contributing to service improvement initiatives.

· Preparing case notes, reports,



funding submissions and other documentation in accordance with organisational and legislative requirements.

· Maintaining compliance with the NDIS Practice Standards, organisational policies, privacy legislation and workplace health and safety requirements.

Qualification/Experience Required:

· Diploma of Community Services (minimum qualification).

· At least two years of relevant experience in community services, disability support or a related field.

· Demonstrated experience working with people with disabilities and other vulnerable groups.

Essential Requirements:

· Strong communication, interpersonal and case management skills.

· Ability to work independently and collaboratively within a multidisciplinary team.

· Sound knowledge of person-centred practice, safeguarding principles and community support services.

· Current National Police Check (or willingness to obtain).

· Current Working with Children Check (where applicable).

· Current First Aid and CPR Certificate (or willingness to obtain).

· Current Australian Driver's Licence.

Employment Details

· Salary: $80,000 - $85,000, plus superannuation.
