About the Role

- Assist participants to access their community and attend appointments

- Deliver clinical and personal care: manual handling, hoisting, continence, PEG feeding, medication administration

- Support daily living: hygiene, meals, mobility, transport

- Monitor health changes and follow clinical care plans

- Maintain accurate case notes and incident reports

- Collaborate with families, allied health and nursing teams

- Ensure a safe, clean, and engaging home workplace

About You

- Minimum 3-6 months’ experience or Min Certificate III/IV in Disability, Individual Support, Nursing, or related fields.

- Experience with high physical needs and clinical care

- Confident in manual handling, medication administration, and hoist transfers

- NDIS Worker Screening, First Aid, valid Driver’s Licence, and insured vehicle
