HelloDR Esk is the local medical centre at 98 Ipswich Street, Esk, right in the heart of the Somerset region. We're the trusted health hub for Esk and the surrounding rural communities, offering GP consultations, bulk billing (eligibility criteria may apply), DAME (Designated Aviation Medical Examiner) services, travel vaccinations, skin cancer checks and onsite audiology through our local hearing clinic.
We're a growing, community-focused, small-town clinic with genuine reach - the kind of place where staff know their patients by name. With a solid emphasis on patient-centered care, our team of healthcare professionals works collaboratively to create a welcoming and supportive environment for everyone who walks through our doors.
The HelloDR Esk operates during the following hours:
- Monday to Friday: 8:30 AM – 6:00 PM
- Saturday: Closed
- Sunday: Closed
The Role
Make a meaningful difference by supporting patients facing mental health, social, and community challenges. As our Social Worker, you'll provide compassionate psychosocial support, helping patients navigate life's challenges while connecting them with the services and resources they need.
Working closely with our GPs and allied health team, you'll contribute to holistic,
patient-centred care through Medicare Mental Health Treatment Plans and other referral pathways, ensuring each patient receives the right support to improve their overall wellbeing.
Key Responsibilities
- Provide short-term counselling and psychosocial support to patients referred by our GPs
- Support patients navigating housing, financial, family or community pressures alongside health concerns
- Coordinate referrals to external mental health and community services
- Maintain accurate, sensitive clinical documentation
- Advocate for patient wellbeing
About You
- Bachelor's degree in Social Work (or equivalent) with AASW eligibility
- Accredited Mental Health Social Worker status highly regarded
- Experience providing trauma-informed, person-centred care
- A steady, grounded presence — this role sees people on hard days
- Compassionate and patient-focused
