We are currently seeking passionate, reliable, and person-centred Support Workers and an experienced Support Coordinator to join a growing team supporting NDIS participants with a range of physical, neurological, psychosocial, and complex support needs across Melbourne's South Eastern Suburbs and surrounding areas.

This is an prospect to work alongside participants, families, and allied health professionals to create meaningful outcomes and improve quality of life.

Minimum Requirements

- At least 12 months' experience working in disability support, community services, aged care, mental health, allied health, or a related field
- NDIS Worker Screening Check (or willingness to obtain)
- Current First Aid and CPR
- Working With Children Check (where applicable)
- Driver's Licence and reliable transport
- Professional references
- Strong written and verbal communication skills
- Ability to write clear, detailed progress notes and incident reports

Support Worker Responsibilities:

- Personal care and daily living assistance

- Medication prompting or assistance (where appropriate)
- Behaviour support implementation
- Maintaining detailed progress notes and handovers
- Working collaboratively with families and professionals

Support Coordinator Responsibilities:

- Coordinating NDIS-funded supports and services
- Liaising with participants, families, providers, and allied health teams
- Assisting participants to achieve their goals
- Building sustainable support networks
- Problem-solving and advocacy
- Supporting participants through changes, challenges, and transitions

Highly Regarded:

- Excellent spoken and written English
- Strong documentation and report-writing skills
- Ability to communicate effectively with participants, families, support teams, and allied health professionals
- Experience supporting participants with complex disabilities, epilepsy, neurological conditions, mental health challenges, and behaviours of concern
- Knowledge of the NDIS and person-centred practice
- Qualifications or studies in disability, nursing, psychology, social work, occupational therapy, education, community services, or related fields
