Chorus is seeking a Customer Partner to empower seniors and people living with disability within a local community. You will work with a small team to assess customers, onboard those who can advantage from Chorus services, and maintain solid relationships with referral partners and health providers.
You’ll split time between direct service delivery and supporting customers and the local support worker community.
