HelloDR Esk is the local medical centre at 98 Ipswich Street, Esk, right in the heart of the Somerset region. We're the trusted health hub for Esk and the surrounding rural communities, offering GP consultations, bulk billing (eligibility criteria may apply), DAME (Designated Aviation Medical Examiner) services, travel vaccinations, skin cancer checks and onsite audiology through our local hearing clinic.

The HelloDR Esk operates during the following hours:
Monday to Friday: 8:30 AM – 6:00 PM
Saturday: Closed
Sunday: Closed
The Role
Join our chronic disease management team and help empower patients to live healthier, more confident lives. As our Diabetes Educator, you'll provide personalised education, coaching, and ongoing support to people living with diabetes through one-on-one consultations and group education sessions.
You'll help patients understand insulin therapy,
blood glucose monitoring, nutrition, and lifestyle changes while working closely with our GPs and allied health team to deliver coordinated, patient-centred care that improves long-term health outcomes.
Key Responsibilities
Deliver individual and group diabetes education and self-management support
Support patients starting or adjusting insulin and other diabetes therapies
Work with GPs and practice nurses on shared care planning for diabetic patients
Keep up to date with NDSS programs and referral pathways
Support chronic disease management
About You
Credentialed Diabetes Educator (CDE) status with ADEA, or actively working towards it
Current AHPRA registration or relevant qualification
Genuine patience and a knack for explaining complex things simply
