Registered Nurse / Case Manager – Aged Care

We are looking for an experiencedRegistered Nurse (RN) / Case Managerto join our growing aged care team in a combined clinical and care coordination role.
This is an excellent opportunity for a qualified RN who enjoys bothproviding clinical support and managing the overall care journey of aged care clients.
About the Role
As an RN / Case Manager, you will combine your clinical nursing expertise with case management responsibilities to ensure clients receive safe, personalised and coordinated care.
You will work closely with clients, families, support workers, GPs, allied health professionals and other service providers to assess needs, develop care plans and coordinate appropriate supports.
Key Responsibilities
Conduct client assessments and monitor changing health and care needs
Develop, implement and review individualised care plans
Coordinate aged care services and support arrangements
Provide clinical advice and guidance to clients, families and care staff
Monitor client wellbeing and identify health or safety risks
Liaise with GPs, allied health professionals and other healthcare providers
Coordinate and review services to ensure client needs are being met
Maintain accurate clinical and case management documentation
Support quality, compliance and person-centred care practices
Respond appropriately to incidents, changes in condition and emerging client needs
Advocate for clients and promote independence, dignity and choice
About You
We are seeking someone who can confidently combineclinical nursing skills with strong case management and communication abilities.
You will have:
CurrentRegistered Nurse registration with AHPRA
Previous experience in aged care, community nursing, case management or care coordination
Strong understanding of person-centred care
Excellent communication and relationship-building skills
Strong assessment, documentation and organisational skills
Ability to work independently and manage multiple client needs
A genuine passion for supporting older Australians
Desirable
Experience with Home Care Packages and/or Support at Home
Previous case management or care coordination experience
Experience working with aged care clients in the community
Current Australian driver's licence and reliable vehicle
