About the Role:
Miracle Health Services is seeking a dedicatedCare Partnerto join ourHome Care Programteam inWangaratta, VIC.
In this role, you will oversee a caseload of clients, advocating for their independence and enabling them to remain safely in their own homes.
You will facilitate personalised discussions and create tailored care plans that align with their needs, ensuring they receive the best support through theirHome Care Package.
Key Responsibilities:
Conduct regularhome visitsto assess and support clients.
Performinitial assessmentsfor new clients, identifying their needs and making appropriate referrals or recommendations.
Develop, implement, monitor, and reviewindividualised, client-focused care plans.
Work closely withCare Coordinatorsto organise and deliver services that align with each client's care plan.
Liaise withhealthcare providers, allied health professionals, social workers, and community servicesto provide holistic support.
Managefinancial aspectsof care planning, ensuring compliance withHome Care Package funding guidelineswhile maintaining quality service delivery.
Supportcase discussions, case conferences, feedback reviews, incident management, and complaint resolution.




Keepaccurate recordsof client assessments, care plans, services provided, and any changes or incidents.
Contribute toteamwork and continuous improvement initiatives.

You'll be a great fit if you have:
Aclinical backgroundas aSocial Worker, Enrolled Nurse (EN), Registered Nurse (RN), Allied Health Practitioner, or Case Manager(highly desirable).
Experience managingHome Care Package clients with complex needs.
Robust knowledge ofsafety practicesin theAged Care or Health Sectors.
Proficiency inassessments, care planning, service implementation, evaluation, and documentation, followingConsumer Directed Care principles.
Excellentcommunication skills, allowing you to liaise effectively with clients, service providers, and healthcare professionals.
A deep understanding ofsafety, equal opportunity, and legal compliancein client care.

Essential Requirements:
Acurrent Working With Children Check(or willingness to obtain one).
The ability topass a national police clearance.
CurrentFirst Aid and CPR certification(or willingness to obtain).

Australian working rights.
Avalid Victorian driver's license and access to a reliable, registered vehicle.
