About the Role:
We are looking for a passionate and experiencedElder Care Team Leaderto lead the delivery of culturally safe, person-centred support for Aboriginal and Torres Strait Islander Elders.
In this rewarding role, you will help Elders and their families navigate the aged care system with confidence, respect and dignity.
You will ensure they understand their rights and entitlements, access appropriate services, and are supported at every stage of their aged care assessment and service journey.
Key Responsibilities:
• Support Elders and their families through the full aged care journey, including My Aged Care registration, assessment preparation, care planning, and ongoing reviews to ensure smooth and informed access to services
• Act as a strong advocate for clients by ensuring culturally safe, trauma-informed, and person-centred care is delivered across all interactions with assessors, service providers, and support agencies
• Work closely with aged care assessors and service providers to coordinate assessments, contribute to care discussions, and ensure support plans reflect the individual needs, goals, and cultural values of Elders
• Educate and guide clients and families on aged care options, service entitlements, contractual arrangements,
and associated costs to support informed decision-making and reduce system complexity
• Plan, deliver, and contribute to community-based health promotion and wellbeing activities focused on healthy ageing, chronic disease prevention, dementia awareness, nutrition, and social connection for Elders
Essential Experience & Skills
• Strong interpersonal and communication skills with the ability to build trust, rapport, and respectful relationships with Aboriginal and Torres Strait Islander Elders, families, and communities.
• Solid understanding of the aged care system, including My Aged Care processes, assessment pathways, service navigation, and client entitlement frameworks
• Demonstrated ability to work in culturally safe, trauma-informed environments with sensitivity to Aboriginal and Torres Strait Islander histories, cultures, and community needs
• Strong advocacy skills with the ability to represent client needs effectively, resolve service issues, and coordinate across multiple stakeholders and service providers
• Excellent organisational and case management skills including documentation, record keeping, reporting, and maintaining accurate client information in line with compliance standards
• Ability to work independently with initiative, while also contributing effectively within a multidisciplinary, community-focused team environment
Qualifications & Requirements
Qualification in health, community services, or related field (desirable)
Current First Aid Certificate
Valid National Criminal History Check
Current Ochre Card clearance (NT Working with Children/People check)
Full Australian working rights
