12 monthfixed-term role, covering parentalleave
Workingfull-timehours,Monday toFriday,based at our West Perthoffice
Flexible working arrangements,including working from home
We attend the office (or site) aminimum of three days per week
About the role
The Aged Care Navigation Specialist(ACNS) will be empowered to provide information,supportandadvocacy to all customers as they navigate the complex aged caresystem.
The ACNS works with customers to understand theirneeds,identifyavailable options, and make informed choices abouttheir care.
Duties will include:
Provide customers theinformation andassistancetheyrequireabout the aged caresystem, including eligibility requirements, services andsupports across the continuum of agedcare
Manage waitlists and vacancyboards in Epicor (CRM system), including working closely withHome Care, Village and Facility Managers, including looking atHome Care as an interim option for customersonawaitlist
Engage and develop strongrelationships with internal teams including Customer EngagementNurses, Case Managers and other healthcare professionals,tofacilitateand optimise the client / residentonboarding process
Monitor and communicate changesin legislation that affect subsidies and/orfunding
Review the NSAF and / orIntegrated Assessment Tool, includingidentifyingtheprospective AN-ACC classification,



and highlighting anyconcerns in the delivery of care and services either in thecustomer's home or in one of our Villages orFacilities.
About you
You are a highly experienced customerservice professional and bring a robust understanding of the agedcare industry.
You can relate to a diverse range of stakeholdersand have:
Ability to develop relationshipswith our clients/residents, their families/ representatives,and staff
Excellent customer serviceskills, a passion for the wants,desiresand need of olderAustralians
Excellent verbal and writtencommunication skills
Strong understanding of MSOffice suite of products as well as database/CRMsoftware
Ability to work independentlyand autonomously, but also as part of ateam
Knowledgeof Home Carefunding programs, Medicare claims, AN-ACC classification andthe My Aged Care Service and Support Portal isdesirable
