About the role:
Provide diabetes care to people living with diabetes attending Western Health at any of their campuses.
This can include inpatient, outpatient and community care.
Key responsibilities:
Perform assessment and management for people living with diabetes.
This may include carers or family members
You will be responsible for performing advanced assessments, prescribing, ordering pathology or other roles undertaken by registered Nurse Practitioners
You will liaise with Endocrinology Medical Team, Diabetes Clinical Nurse Consultants, ward nurses and allied health professionals
