About us:
The Geriatric Evaluation Rapid Intervention (GERI) and Residential Aged Care Facility Acute Support Service (RaSS) is an innovative service dedicated to improving care for older people across Central Queensland.
Operating as an Emergency Department substitution model, the service helps reduce unnecessary hospital presentations through rapid assessment, multidisciplinary care and timely access to community-based support.
Working closely with Residential Aged Care Facilities, GERI/RaSS provides expert clinical advice, coordinated care and in-reach services that support early discharge, helping older people receive the right care in the right place while enhancing patient outcomes and reducing pressure on emergency services.
About the role:
Deliver advanced geriatric nursing care by conducting comprehensive assessments, managing complex patient needs and coordinating care for older people presenting to the Emergency Department.
Provide clinical leadership and support by mentoring nursing staff, acting as a role model and guiding the delivery of safe, high-quality, evidence-based care.
Coordinate multidisciplinary care to ensure timely assessment, intervention, discharge planning and improved outcomes for frail older patients.
Drive quality improvement and innovation by participating in clinical audits, research, policy implementation and continuous improvement initiatives to enhance patient care.
About you:
Current AHPRA registration as a Registered Nurse,
with a relevant postgraduate qualification in geriatric nursing (or willingness to complete one within an agreed timeframe).
Advanced clinical expertise in assessing, planning, delivering and evaluating person-centered care for older people in complex healthcare settings.
Strong leadership and mentoring skills, with the ability to guide, support and develop nursing staff while promoting excellence in clinical practice.
Commitment to quality improvement, using clinical judgement, data and evidence-based practice to enhance patient outcomes and drive continuous service improvement.
What we offer:
Total remuneration value of up to $69,965 p.a. is comprised of:
An hourly rate between $***** - $***** p.h.
*****% employer superannuation contribution
17.5% leave loading
