We areseeking an experienced and passionate Nurse to join our CommunityTreatment Team at Cockburn Mental Health.
As a ClinicalNurse within the Community Treatment Team, you willdeliver advanced nursing care and care coordination for patientsrequiring treatment outside of the hospital environment.
Working aspart of a multidisciplinary team, you will assess, plan, implement,and evaluate person-centred care while supporting patients andtheir families through their healthcare journey.
Thesuccessful applicant shall be part of a multidisciplinary teamprovides clinical and professional expertise to ensurecomprehensive evidence-based nursing recovery focused care isdelivered to patients.
Facilitates and promotes patient safety andquality of care in the community setting.

Across Fiona Stanley Fremantle Hospital Mental Health(FSFH Mental Health) and Peel and Rockingham Kwinana Mental Health(PaRK Mental Health), specialist services are provided across theSouth Metropolitan region, supporting consumers with accessible,collaborative and high-quality mental health care.
SMHSMental Health will soon open a current state-of-the-art facility atFremantle Hospital designed to offer expanded and modern mentalhealth services for our community.
The three-storey space willprovide an additional 40 mental health beds and a 24-hour MentalHealth Crisis Centre, ensuring compassionate and innovative carefor those who need it most.
CockburnHealth is a stand-alone 75 bed inpatient facility.
Thehospital is non-authorised hospital, and for voluntary femalepatients only.
FSH Cockburn Health provides inpatient servicesfor:
Women'sMental Health.
EatingDisorders.
Alcoholand Other Drug (AOD) Behavioural and WithdrawalManagement.
