About the Role
This is a rewarding part-time temporary chance until 30th June **** (60.8 hours per fortnight) based in Wangaratta, to join our newly funded Seniors Lifestyle Victoria pilot program.
As a Care Coordinator, you will work directly with older people participating in the program.
Using a person-centred, strengths-based approach, you will undertake intake, assessment, care planning and ongoing support, helping participants to build confidence, improve wellbeing, and remain living independently in the community.
This role focuses on early intervention and prevention, supporting older people to reduce, slow or reverse mild to moderate frailty, improve their ability to manage their own health, and reduce avoidable emergency department presentations and hospital admissions.




This role is classified under the Community Health Centre Stand Alone Services Social and Community Service Employees Multi Enterprise Agreement **** at a Level 3 ($************* per hour + superannuation.
Skills & Experience
To thrive in this role, you'll bring:
Tertiary qualifications in Social Welfare, Community Services, Aged Care, Disability, Mental Health or relevant equivalent studies (minimum Certificate IV level)
Demonstrated skills and experience in using a person-centred care approach
Demonstrated skills and experience in working with older people from a range of backgrounds, and an understanding of the barriers they may experience in accessing health care
