We are seeking a compassionate
Enrolled Nurse (Care Partner)
to deliver high-quality, person-centred care in clients' homes. This role combines hands-on clinical care with monitoring client wellbeing to ensure safe, consistent and responsive care.

About the Role
The
Care Partner
delivers high-quality, person-centred care in clients' homes, providing clinical support, monitoring wellbeing and maintaining accurate documentation to ensure safe, responsive and consistent care.
Key Responsibilities
Deliver personal care and in-home support in line with care plans, promoting independence and wellbeing
Monitor client wellbeing and escalate clinical concerns,



risks or deterioration via established escalation pathways
Contribute clinical observations to support care plan development and review
Support implementation of care plans, reinforcing safe, consistent and person-centred practices
Maintain accurate, timely clinical documentation to support compliance and continuity of care
Identify and report incidents, hazards and safeguarding concerns
Build trusted, professional relationships with clients, families and care teams
About You
You are a compassionate and reliable care professional who takes pride in supporting others to live with dignity and independence. You bring strong communication skills, sound judgement and a genuine commitment to secure, person-centred care.
To Be Successful You Will Have
Relevant qualification or experience in in-home or personal care
Current National Police Clearance (or willingness to obtain)
Working With Children Check
NDIS Worker Screening Clearance (or willingness to obtain)
A valid driver's licence and reliable vehicle
Evidence of current mandatory immunisations
Full right to work in Australia
