About us
The Corrections and Justice Services (CJS) group, comprising Corrections Victoria (CV), Justice Health and Justice Services, plays an integral role in developing safer and more resilient communities and in delivering a trusted justice and community safety system.
CV is responsible for the implementation of policies, programs and services that ensure the safe and secure containment of prisoners, and that seek to rehabilitate offenders by addressing the underlying causes of offending behaviour.
As part of the CJS group, the Dame Phyllis Frost Centre (DPFC) is a 649-bed maximum-security facility for female offenders located within the West Metropolitan Area, Department of Justice and Community Safety.
What you will do
As assessment and transition coordinator, you will help women in custody prepare for life after prison through assessment, planning and support.
You will play an important role in transition planning, risk assessment and connecting women with the right services before release.
You will:
* complete prisoner risk and needs assessments and support case management reviews
* coordinate pre-release services and refer eligible women to reintegration programs
* deliver individual and group sessions with service providers and prison staff
* maintain accurate records, data and program schedules to support quality services
* support parole planning and share information with community corrections to improve transition outcomes.
About you
A tertiary qualification in a relevant discipline is highly desirable.




Skills and experience in human services, welfare practice or related fields are also highly desirable.
Experience using evidence-based approaches with people who have complex needs is highly regarded.
You should also have:
* strong knowledge of transition and reintegration needs for women in custody
* experience using risk assessment tools and supporting people with complex needs
* sound judgement, critical thinking and the ability to solve problems
* effective communication skills, flexibility and a commitment to inclusive teamwork.
