ABOUT THE ROLE
As a TCP Key Worker, you will play a vital role in supporting children and young people to transition from residential care into more stable, nurturing living environments. Using Flexible Funding Packages, including Targeted Care Packages (TCPs), you'll deliver tailored, needs-based support that empowers positive outcomes—whether through family reunification, kinship care, home-based care, or independent living.
In this role, you'll lead the implementation and case management of individualised care plans, working closely with internal teams and external stakeholders to ensure each child's needs, goals and cultural supports are met. Your work will directly contribute to creating safer, more positive care experiences and long-term stability for young people.
KEY RESPONSIBILITIES
Deliver therapeutic support to children, young people and families to achieve case plan goals
Act as key contact, coordinating care and leading collaboration with families, carers and professionals
Build and manage care teams to support complex needs and improve outcomes
Implement and monitor targeted care plans, ensuring a holistic, client-focused approach
Support cultural needs, including Aboriginal cultural support plans
Maintain accurate client records in line with DHHS and confidentiality requirements
Participate in team meetings, supervision and care planning discussions
Complete duties aligned with Flexible Funding Packages
This is a Fixed Term Part-Time opportunity (30.4 hours per week) based in Bendigo, VIC, until 1
st of July ****.
Key Selection Criteria
To be successful in this role, you will have:




Relevant qualifications in Social Work, Social of Behavioural Science, Community Services, or other relevant discipline
An awareness, understanding and sensitivity to homelessness issues, the services and supports they may access, and the ability and experience to communicate with a diversity of people from various cultures.
Knowledge of the Community Services sector with direct and applicable work experience
Awareness of Aboriginal culture and issues impacting children, young people and families
Demonstrated experience in community service or housing sectors.
