Role Description
Pilbara Support Network (PSN), a subsidiary of Ngarliyarndu Bindirri Aboriginal Corporation (NBAC), is seeking a compassionate and motivated Part-Time Case Manager to join our Newman team on a 48-month fixed-term contract. This rewarding role provides the opportunity to support individuals and families through person-centred case management, advocacy and service coordination. You'll work closely with clients to identify their goals, connect them with appropriate services and provide ongoing support to achieve positive outcomes. We recognise the importance of work-life balance and offer flexible part-time hours, with school-friendly hours able to be discussed with the successful candidate, subject to operational requirements.
Key Responsibilities
Build positive relationships with clients through strengths-based, person-centred case management.
Complete client assessments and develop tailored support plans.
Coordinate referrals and connect clients with appropriate community and support services.
Advocate for clients and assist them in achieving their personal goals.
Maintain accurate, confidential client records and prepare reports as required.
Collaborate with government agencies, community organisations and internal stakeholders.
Monitor client progress and review support plans to ensure effective service delivery.
Contribute to continuous improvement and culturally safe service delivery.
Qualifications & Experience
Experience in case management, community services, social work or a similar human services role.
Strong communication, interpersonal and relationship-building skills.
Excellent organisational skills with the ability to manage competing priorities.
A commitment to culturally safe, trauma-informed and client-centred practice.
The ability to work both independently and collaboratively within a supportive team.
A qualification in Community Services, Social Work, Psychology or a related discipline (or equivalent industry experience).
A current C Class Driver's Licence.
A current National Police Clearance (or ability to obtain one).
