About the Role
Walk alongside families to shape better health outcomes for the next generation.
Aboriginal communities in Adelaide's north deserve health care that meets them where they are. Care that feels safe, respectful, and grounded in culture. As an Aboriginal Health Navigator with the Northern Adelaide Local Health Network (NALHN), you'll be the connection. You'll walk with patients and families, help them feel confident in a system that can be hard to navigate alone, and make a real difference in the health and wellbeing of Aboriginal and Torres Strait Islander children and mothers birthing Aboriginal and Torres Strait Islander children.
What You'll Be Doing
Based within the NALHN Continuity of Care Program (within the Women and Children's Division),
and working across Adelaide's northern metro region, you'll support Aboriginal patients to access, understand, and engage with health services during pregnancy, and through early years of a child's life (0 - 4 years). This is a
full-time role
, long-term contract role through to
30 June ****
(part time hours considered).
Provide culturally responsive navigation support
to Aboriginal and Torres Strait Islander patients and their families, ensuring they feel secure, respected and understood
Build strong relationships
with clinical teams, community organisations, and external services to coordinate care pathways
Walk alongside families
to help them attend appointments, understand their care plans, and stay connected with follow-up services
Promote cultural
