We are seeking an experienced and motivated
Support Services Facilitator

About the Role
Reporting to the Support Services Manager, you will act as a key point of contact for participants, families, nominees and stakeholders, ensuring services are delivered in accordance with participant goals, NDIS plans and organisational requirements.
As a
Support Services Facilitator
, you will coordinate supports, maintain strong stakeholder relationships, oversee participant documentation and contribute to positive outcomes through proactive problem-solving and service delivery excellence.
Key Responsibilities
Coordinate and monitor participant supports and services in line with NDIS plans and goals.
Build and maintain positive relationships with participants, staff, families, service providers and community stakeholders.
Ensure participant support plans and service plans are implemented effectively.
Complete participant documentation, case notes, reports and NDIS-related records accurately and on time.
Respond to participant enquiries and stakeholder communications professionally and efficiently.
Support quality improvement initiatives and identify opportunities to enhance service delivery.
Attend participant meetings, reviews, networking events and training as required.
Maintain compliance with NDIS requirements, organisational policies and relevant legislation.
Contribute to a collaborative and positive team culture.
About You




You are a highly organised and people-focused professional with a genuine passion for supporting individuals living with disability.

Experience working within the disability, community services or NDIS sector.
Strong relationship-building and stakeholder engagement skills.
Excellent written and verbal communication skills.
High attention to detail and strong administrative capabilities.
Exceptional time management and organisational skills.
Ability to work independently while contributing positively to a team workplace.
Strong problem-solving and decision-making skills.
The ability to engage, motivate and support Disability Support Workers to deliver high-quality, participant-centred services.
Confidence in providing guidance, coaching and constructive feedback to frontline staff while fostering accountability and skilled growth.
A professional, compassionate and participant-centred approach.
Ability to maintain confidentiality and manage sensitive information appropriately.
A commitment to promoting a positive workplace culture built on collaboration, respect, communication and continuous improvement.
Essential Requirements
Current unrestricted Australian Driver Licence.
Current National Police Check.
NDIS Worker Screening Clearance.
Working with Children Check (Blue Card).
Current First Aid and CPR Certificate.
Comprehensive vehicle insurance.
Relevant NDIS training modules and industry certifications, or willingness to obtain prior to commencement.
Desirable
Certificate III in Individual Support, Community Services, Disability, or another relevant qualification.
Previous experience in Disability Services, Community Services, Support Coordination, Case Management, Service Delivery or a similar role.
