Roles & Responsibilities include:
• Supporting clients to live an active lifestyle and participate in their community through the delivery of a range of person-cantered programs
• Work to behavior support plans by utilizing positive behavior strategies
• Assist and support clients to access the community e.g. driving clients to the shops, appointments, coffee, gym, outdoor activities etc.
• Assist and support clients within their own home e.g. assisting with household duties, personal and hygiene care, cooking etc.
• Maintenance of progress notes and record-keeping
• Medication Management and Administration
• Supporting clients who use personal equipment /aid, this may include walking or standing frames, wheelchairs, hoists
