Position Overview:
We are seeking a skilled and dedicated
Nurse Practitioner
to join our Perth Metro team. In this role, you will provide advanced nursing care, support complex healthcare needs, and collaborate with multidisciplinary teams to deliver comprehensive, patient-centered services.
Key Responsibilities:
Conduct advanced assessments, and diagnoses, and manage acute and chronic health conditions across all age groups.
Prescribe medications, order diagnostic tests, and interpret results to guide evidence-based care plans.
Partner with general practitioners, specialists, and allied health professionals to ensure seamless, integrated care.
Educate and empower patients and their families on health promotion, disease prevention, and self-management strategies.
Participate in community health initiatives to improve population health outcomes within the metro area.
Engage in ongoing professional development to stay updated with advancements in clinical practice.
Maintain thorough documentation of clinical activities in line with organizational and professional standards.
Requirements:
Current Nurse Practitioner registration and endorsement with AHPRA.
Master's degree or higher in Nursing with Nurse Practitioner specialisation and relevant clinical experience.
Expertise in primary healthcare or acute care settings, with strong diagnostic and decision-making skills.
Exceptional communication and interpersonal skills for fostering therapeutic relationships.
Commitment to culturally safe practices, especially when working with diverse populations.
Ability to adapt to dynamic healthcare environments and contribute proactively to team goals.
Valid driver's license and flexibility to travel locally if required.
