About the role
We are seeking a dedicated and compassionate
Community Worker
to join our team at Amity WA on a
full time basis

. In this pivotal role, you will provide essential support and assistance to individuals within the aged and disability support sector. As a Community Worker, you will be instrumental in enhancing the quality of life for vulnerable clients by delivering personalised care, facilitating community engagement, and promoting independence and social inclusion.
Key responsibilities
Provide direct support and assistance to clients in the aged and disability sectors, helping them with daily living activities and community participation
Develop and maintain supportive relationships with clients, understanding their individual needs, goals, and preferences
Assist clients in accessing community resources, activities, and social opportunities to promote engagement and inclusion
Support clients with practical tasks such as shopping, meal preparation, personal care coordination, and mobility assistance
Liaise with families, carers, and other professionals to ensure coordinated and holistic support
Maintain accurate and confidential client records and documentation in compliance with organisational policies
Identify and report any concerns regarding client welfare or safeguarding issues to appropriate supervisors
Participate in team meetings and contribute to the continuous improvement of services
Demonstrate commitment to person-centred care principles and uphold the dignity and rights of all clients
Comply with all relevant legislation, including health and safety, disability discrimination, and privacy requirements
What we're looking for
A genuine commitment to supporting vulnerable people and promoting their independence and wellbeing
Previous experience working in community services, aged care, disability support, or a related healthcare setting
Strong interpersonal and communication skills with the ability to build rapport and establish trust with diverse individuals
Compassion, patience, and emotional resilience when working with individuals facing challenges
Ability to work independently and as part of a team in a fast-paced environment
Problem-solving skills and the ability to adapt to the changing needs of clients
A current valid driver's licence and reliable access to transport
Working With Children Check (WWCC) and/or other relevant background clearances (or willingness to obtain)
First Aid and CPR certification (or willingness to obtain)
Excellent organisational skills and the ability to manage multiple priorities
Commitment to maintaining confidentiality and respecting client privacy
Understanding of and adherence to disability rights, aged care standards, and person-centred practice principles
