reablement and restorative care approachManage client budgets and monitor service delivery outcomesIdentify risks and implement appropriate support strategiesMaintain accurate documentation and compliance with aged care standardsLiaise with internal teams, families and external stakeholdersContribute to continuous improvement and quality initiativesParticipate in reporting, team meetings and organisational change initiativesAbout YouYou are a compassionate and organised professional with strong case management and communication skills.
