You will be based in the community with your clients and join us in the office based in Mulgrave and enjoy free onsite parkingYour main duties will include:Conduct client visits practicing person centred care planning, offering personalised advice and guidance to clients and/or their familiesConduct Annual Reviews, revising Care Plans, Goals and service delivery,



ensuring budget accuracy and maximum utilisationInteracting with internal and external stakeholdersAbout YouClear understanding of Support at Home packages and the recent sector changes, strong understanding of the Aged Care Quality Standards and available community service networksPrevious or current experience in Care Management within Home Care, EN or RN highly desirableHighly organised with the ability to plan, delegate, and manage competing prioritiesDiploma or degree in Health/Allied, or 3+ years' relevant industry experienceCurrent Australian driver's licence and reliable vehicleWhy work for Calvary?
