Our Support Coordination services are designed to help participants make the most of their NDIS plans by connecting them with the right services, providers, and resources to achieve their goals.
The successful applicant will assist participants in understanding and implementing their NDIS plans effectively.
Our team works closely with individuals to identify their needs, explore available support options, and establish service agreements with providers.
We focus on building participants' capacity to manage their supports independently over time.TasksHelping participants understand their NDIS plan and funding allocationsclients and assessing the nature and extent of difficultiesConnecting with service providers and community resourcesCoordinating different supports to ensure seamless service deliveryAssisting with problem-solving and overcoming barriers to accessing servicesEncouraging independence and informed decision-makingAssessing community need and resources for health, welfare, housing, employmentInterviewing, training and other facilities and servicesSupporting families and providing education and care for children and disabled persons in adult service units, group housing and government institutionsLiaising with community groups, welfare agencies, government bodies and private businesses about community issues and promoting awareness of community resources and servicesYou should be able to demonstrate the following qualifications to be considered:Relevant Diploma or higher qualification plus at least one year of work experience in an accredited residential care or disability care program setting.
