You will complete functional assessments, equipment and home modification recommendations, and implement practical intervention plans that align with each person's goals and daily needs.
Managing your own caseload, you will ensure documentation is accurate, outcomes are measurable, and services are delivered within agreed timeframes while working closely with clients, families, carers,



and other stakeholders to achieve meaningful functional outcomes.You will collaborate within a multidisciplinary team while also building robust relationships with referrers, community services, and support networks to ensure coordinated care.
Alongside delivering high-quality therapy services, you will contribute to service growth through stakeholder engagement, help maintain performance targets, and support continuous improvement initiatives.
