Job Description
About the Role
As a Case Manager, you'll:
Conduct comprehensive assessments to understand each client's unique needs.
Collaborate with healthcare professionals and service providers to deliver tailored support.
Build robust relationships with clients and key stakeholders.
Ensure government funding is used effectively and in line with guidelines.
Exceptional organisational skills and ability to self-manage.
Strong communication skills – both written and verbal.
Proficiency in using Microsoft Suite and client management systems.
Commitment to working within a multidisciplinary team.
The Successful Applicant will have:
Current Working with Vulnerable People/Children Check.
Current National Police Check.
Current Australian Driver's Licence.
