Gold Coast Health is seeking a Social Worker (Hospital in the Home) to deliver social work services across hospital, community and home-visiting settings on the Gold Coast and northern NSW region.

You will conduct psychosocial assessments, crisis intervention and discharge planning, applying independent clinical judgement in complex care situations and collaborating with multidisciplinary teams to improve patient outcomes.
