- Pay rate – $48 per hour + superannuation +Km reimbursement

We are seeking an experienced and values driven Care Partner to join our Home & Community team at Port Macquarie. T his is a highly autonomous position, ideal for someone who is self-driven, resourceful, and comfortable working independently.

The successful candidate will be confident in building local networks, driving community engagement, and managing priorities with limited on-site support .

Acting as a community ambassador for Catholic Healthcare, your day-to-day role looks like:

- Providing high-quality care management to support client independence, preferences whilst educating clients and their families on program guidelines, inclusions, exclusions and possible financial contributions
- Identifyingclient care needs, requirements and setting client service expectations within budget guidelines
- Undertaking relationship-centred care planning in accordance with CHL model of care and maintaining care plans that clearly link goals, risks, interventions and services
- Completing comprehensive care reviews, screeningtoolsand risk assessments promptly and accurately to identifyrequired services and referrals needed
- Managing new client sign-ups and onboarding across multiple programs, including Support at Home, CHSP, End of Life (EOL), AT-HM, and Restorative Care.




- Driving stakeholder engagement through presentations at local community events and the coordination of CHL-hosted events, both in person and online.
- Developing and maintaining strong local networks by attending community meetings, building referral pipelines, identifying business development opportunities, and collaborating with external providers.
- Acting as a trusted community liaison, strengthening CHL’s presence and fostering meaningful partnerships across the Port Macquarie region.
- Using screening,assessmentand risk dashboards to identifyhigh riskclients and prioritize timelyreviews
- Managing incidents and complaints within requiredtimeframes, resolving issues wherever possible at first contact.
- Initiating case conferencing to support hospital, rehab or respite discharge planning for wellbeing interventions, serviceadjustmentsand timelyreferrals
- Communicating and coordinating with members of the clinical, allied health and operational teams toidentifyclient care needs, plan interventions, and review outcomes of care
- Managing the implementation of service delivery to clientsin accordance withclient agreements and funding parameters whilst maintaininghigh levelsof customer satisfaction
- Completing accurate and timely documentation and record keeping of client details in the relevant Client Relationship & Management Systems.

T o be successful in this role you’llneed:

- Strong stakeholder management skills and/or extensive case management experience.




- Minimum two years’ experience in Aged Care Industry within Home Care/Community services sector with solid understanding of case management and funding frameworks
- Strong time management andorganisationalskills with ability to prioritise
- Strongcustomer‑serviceorientation and ability to build rapport with diverse clients and supporters.
- Intermediate to advanced skills in use of information systems and software packages such as Microsoft Office
- Demonstrated skills in budgeting orfinancial management
- Current Driver's License and registered motor vehicle
- Experience in Sales/Marketing/Public speaking
