Catholic Healthcare is recruiting a Clinical Care Partner to provide relationship-centred care with a clinical lens for clients living at home in Western Sydney. The role is permanent full time, based at the Hunters Hills office with regular travel to support care in the community.

You will lead care planning, coordinate with the clinical and allied health teams, complete nursing assessments, ensure accurate documentation and billing, and help clients to thrive safely at home; the role aligns
