Clinical Care Coordinator (registered nurse) About the role

As a Clinical Care Coordinator, you will be an experienced Registered Nurse providing essential clinical support to older adults living in their own homes, ensuring protected, effective, and holistic care that promotes wellbeing, independence, and quality of life.

Working closely with the Clinical Services Manager, you will ensure clients receive safe, person‑centred care aligned with their goals, health needs, and funding programs. You will also provide guidance, supervision, and support to a team of Enrolled Nurses.

Key responsibilities

- Provide clinical oversight, supervision, and mentoring for Enrolled Nurses, ensuring safe practice and adherence to professional standards.
- Conduct comprehensive assessments, develop and review care plans, and monitor clients' clinical needs in line with their health goals and personal preferences.
- Work collaboratively with Care Coordinators to ensure clinical interventions align with client needs and funding requirements.
- Deliver direct nursing care, including wound care, chronic disease management, medication management, and health education.
- Respond promptly to changes in client health status, initiating appropriate interventions or referrals as required.
- Maintain strong communication with clients, families, caregivers, allied health professionals, general practitioners, Care Coordinators, and external providers.
- Support the delivery of education and training for team members in areas such as wound care, clinical assessment, medication administration, and infection prevention.
- Regularly review and evaluate care plans to ensure clinical goals and client needs continue to be met.




- Maintain accurate clinical records and documentation and contribute to reporting requirements.
- Participate in ongoing professional development and continuous service improvement initiatives.

Experience and qualifications Essential

- Registered Nurse qualification.
