HammondCare is seeking an experienced Level 1/2 Social Worker for part-time casual work across Northern Sydney sites (Wahroonga, St Leonards, Mona Vale, North Ryde). The role focuses on holistic psychosocial care, complex discharge planning and collaboration within multidisciplinary teams in palliative care and older persons mental health.

Flexible schedule, Fridays required, with ongoing casual options. Essential criteria include a recognised Social Work qualification, AASW eligibility and
