Service Responsibilities
- Assessment and care planning – complete core clinical tasks such as assessments, reviews, referrals, meetings (HOD/MDT/case conferences) and staff education, as part of the multidisciplinary team on site.
- Deliver manual handling training – on‑site training focuses on creating a practical, hands‑on and engaging experience where participants are challenged to complete tasks and demonstrate their knowledge of manual handling.
- Wellness and re‑ablement programmes – flexible hours to deliver individually tailored wellness and re‑ablement programmes aligned with each resident’s goals and needs.
- Home visits – community physiotherapy delivered by supporting people in their homes and in the community to maintain independence and function.
Duties
- Assess, plan and deliver evidence‑based, goal‑oriented, and outcome‑driven therapy.
- Build a vibrant wellness culture that enhances resident outcomes and engagement.
- Deliver wellness and re‑ablement programmes.
- Build strong client relationships.
- Deliver therapy through 1:1s and group programmes.
- Document clearly and efficiently.
- Collaborate with clinical and multidisciplinary teams.
Skills & Experience
- AHPRA registration.
- National Police Check.
- Driver’s licence and reliable car.
- NDIS Workers Check and orientation.
- COVID‑19 and flu vaccination.
