About The Organisation

We are currently seeking experienced Support Workers to join our growing team across the Perth metropolitan region. We have a range of shifts available, including morning, afternoon, active night and sleepover shifts. Please indicate your availability and preferred shift type in your application.

Key Responsibilities

- Support participants to work towards achieving their individual goals in line with their support plans
- Assist with daily living activities, including personal care, cooking, cleaning, shopping and community access
- Support participants to build confidence, independence and life skills
- Assist with medication administration in line with organisational policies and training
- Implement positive behaviour support strategies
- Encourage participation in age-appropriate activities, employment and social engagement
- Maintain a safe, respectful and supportive environment at all times
- Monitor participant wellbeing and report any concerns to the Manager
- Accurately complete required documentation, progress notes and communication records
- Participate in individual planning processes and monitor outcomes
- Liaise with families, carers, case managers, allied health professionals and community agencies
- Work in line with NDIS standards, WHS guidelines, privacy and confidentiality requirements
- Collaborate with team members to ensure consistent and high-quality service delivery

Skills & Experience

- Qualification preferably in community/disability/individual support/aged care/mental health and/or relevant experience
- Experience working with clients with challenging behaviours




- Well-developed verbal and written communication skills
- Current Driver Licence and reliable vehicle
- 100-point ID check
- Valid visa/working rights in Australia

Shift Timings:

Tue- 6:00 AM to 9:00 AM

Thu- 6:00 AM to 9:00 AM

Fri- 1:00 PM to 4:00 PM

Sat- 6:00 AM to 10:00 AM
