We're looking for a Support Coordinator (CoS) to join us in Geelong and the surrounding region. If you know the NDIS inside out, love working with people and want the freedom to do your job well without being micromanaged - keep reading.

WHAT THE ROLE LOOKS LIKE
You'll work alongside NDIS participants to help them understand their plans, connect with the right supports and build the skills and confidence to navigate the system on their own terms. Some days that's practical and process-driven. Some days it's sitting with someone in a really hard moment and helping them find a way through.
Day to day, you'll be:
- Helping participants understand and get the most from their NDIS plans
- Connecting people with services, community supports and informal networks that fit where they live and how they want to live
- Coordinating complex, multi-provider supports alongside mainstream services
- Monitoring plan budgets across a 12-month cycle
- Completing service agreements, referrals, risk assessments and NDIA reports
- Supporting participants through plan reviews - developing goals, preparing documentation, advocating for what they need
A CoS is not a rostering service,
a personal care worker, or a paid advocate.
This work is about building independence, not dependency.

ABOUT YOU
You'll feel at home here if:
- You have a Diploma or Certificate IV in Community Services, Community Development, Disability, or equivalent relevant experience
- You have solid, practical knowledge of the NDIS
- You're experienced supporting people with mental health and/or psychosocial conditions - Recovery Coaching background is a bonus
- You're warm, transparent and honest communicator - especially under pressure
- You can manage your own priorities and workflow without close supervision
- You genuinely believe in a strengths-based, person-centred approach
- You have experience engaging with inter-agencies and local disability networks
Training in Person-Centred Thinking, ABCD or Art of Hosting is a plus

HOW PAY WORKS
This role is paid under the Social, Community, Home Care and Disability Services Industry Award (SCHADS Award MA000100) at the appropriate classification level based on your qualifications and experience.
