We are currently recruiting on behalf of a respected not-for-profit organisation committed to delivering culturally responsive, person-centred care and support that enables older Australians to live independently and with dignity.

We are seeking an experienced Registered Nurse with a strong Care Advisory background within Home Care Packages (HCP) and experience supporting the transition to Support at Home (SAH) to join a welcoming and supportive team.

This is a full time, 3-month locum position based Monday to Friday in the Brunswick office, with an immediate start available. There is also the potential for the role to transition into a permanent position for the right candidate.

This opportunity is ideal for a clinically experienced and compassionate professional who enjoys building relationships, coordinating care and supporting older people to remain independent while helping shape the future of aged care through the transition to Support at Home.

Key Responsibilities

- Conduct comprehensive clinical assessments and develop tailored care plans
- Provide clinical oversight and coordinate safe, high-quality care delivery
- Monitor and review care plans in line with changing client needs
- Collaborate with GPs, allied health and multidisciplinary teams




- Ensure accurate documentation and compliance with HCP and SAH requirements
- Provide education, guidance and support to staff, clients and families
- Promote client independence, wellbeing and person-centred outcomes

Key Requirements

- Bachelor of Nursing and current AHPRA registration
- Experience in a Care Advisory or community aged care role
- Robust understanding of Home Care Packages and the transition to Support at Home is essential
- Knowledge of aged care systems and funding models
- Understanding of the challenges experienced by older people, families and carers
- Demonstrated commitment to person-centred care
- Excellent communication and stakeholder engagement skills
- Ability to work collaboratively within multidisciplinary teams
- This role works closely with older people from the Greek community. To provide culturally responsive, person-centred care, fluency in Greek is essential.
