Role Summary:
The Client Intake Coordinator engages new and prospective clients to facilitate their access to disability services. A background in social work or community services is essential to enhance community accessibility. The role involves timely follow-up with internal stakeholders and ensuring a positive client experience during service initiation. Candidates must hold a Social Work degree or Community Services certificate/diploma.
Responsibilities:
- Serve as the primary phone contact for clients and families.
- Assist with client assessment, onboarding, and needs reviews.
- Process intakes and referrals for aged care and allied health clients efficiently.
- Respond promptly to client inquiries.
- Complete onboarding documentation and administrative tasks.
- Build relationships with stakeholders to improve service delivery.
- Coordinate services by matching staff skills to participant needs and managing bookings using IT systems like SharePoint and MS Office.
- Implement and monitor support plans, facilitate audits, maintain compliance, and respond to referrals swiftly.
- Manage time effectively, support clients in accessing community resources, and report issues promptly.
- Collaborate with managers/coordinators to ensure timely onboarding and documentation.
- Organize community activities to engage clients and meet KPIs.

Key Requirements:
- Minimum 1+ years' experience in the NDIS industry.
- Robust communication skills and ability to build effective working relationships.
- Ability to identify and manage operational challenges.
- Registered Nurse qualification is an advantage.
- Current NDIS Worker Screening Check required at commencement.
